Insurances for which we may be contracted providers:

Please note that our practice periodically must close to new patients in order for us to best serve our current patients. We are currently closed to new Medicare patients, except for family members in the same household as current patients, or current patients converting to Medicare as they age. This list is subject to change at any time, and not a guarantee that we accept the following plans at this time. The most current information is available by calling our office. Please bring your Benefits Booklet to your visit in case you need referral to a lab, imaging, or pharmacy.

If you have a different plan from the list above, We can still provide you care on a "non-contracted provider" basis. What this means is that we will still bill your insurance for you; but you may have a larger co-payment at time of service; and you will be billed for any amount that your insurance company does not pay, which you then must pay personally. Alternatively, you can pay us in full at the time of service and we can have your insurance company pay you. This probably does not affect your ability to use your insurance with labs, x-ray, other specialists or hospitals. If this plan is acceptable to you, all you need to do is bring your Insurance Benefits Booklet with you to your appointment (if you can't find yours, ask your employer for another copy), and be prepared to make a larger co-payment as specified in the Benefits Booklet. (It might be as little as $10)

All co-payments, deductibles and fees for non-insured services are due at the time of your appointment.

Insurance and Health Plans
We will bill insurance carriers for you and collect their portion as a courtesy to you. You are ultimately responsible for all professional fees regardless of your insurance coverage status. Your insurance benefits are a matter between you, your insurance carrier and your employer. If your insurance company keeps denying coverage for benefits you believe you are owed, you can always file a complaint with the California State Insurance Commissioner. 50% of all complaints are resolved in favor of the patient!

We do not accept MediCal or Health Plan of San Mateo except as a secondary insurance for our Medicare patients. We do not accept most Covered California plans. You must provide us with your health plan card and/or all information necessary for us to bill your insurance carrier, or the fee for your visit will be due at the time of your appointment. You may need to obtain pre-authorization from your carrier in order to avoid unnecessary cost to you for care that may not be included in your plan. Please familiarize yourself with what your plan coverage is, or contact your personnel department or insurance carrier.

Auto Accidents and Personal Injuries
If your problem is due to an auto accident or other "personal" injury involving another party please let us know at the time of making your appointment. We do NOT handle auto-accident or Personal Injury Policy (“PIP”) insurance. We do not accept "liens" (i.e. "IOUs" from your attorney). Examples include accidents at work, injuries and conditions acquired at businesses and homes, assault claims, accidents in the home, product defect accidents, and holiday accidents. Since your injuries may involve different insurance companies with whom we are not providers, payments for medical care are due at the time services are rendered. If you come for a visit and your medical problem is of this type, you must pay in full at the time of the visit, and we will give you a receipt which you can submit for reimbursement to the auto-insurance or PIP company handling it. We can provide you with copies of reports and paperwork for a minimal fee if you need them. Please call our billing office with any questions. We will suggest specialists or testing based on medical necessity in our opinion. It is your responsibility to find the X-ray, lab, specialist, chiropractor, acupuncturist or other specialist that will accept your insurance. Please call your insurance company, or the specialist to whom we refer you, with any questions about coverage. If these policies are not acceptable to you, you will need to find a different doctor or urgent care clinic for your auto accident or personal injury claim, though we can still see you for your other regular medical needs.

Workers Compensation
Our practice does not provide care for Workers Compensation cases (any injury, condition or complaint acquired-at or related-to your employment). Ask your employer for a referral to a Workers Compensation clinic, or look in the Internet or Yellow Pages for the nearest Workers Compensation clinic near you (also known as "industrial" or "occupational medicine" clinics). If you wish supplemental care, or a second opinion on the care you are receiving, we can see you with payment in full due at the time of service, as your regular insurance coverage does not apply. You must notify us of such at the time you make your appointment.
